I’m having trouble signing the Release of Information form, what can I do?

We recognize that you may not be able to sign the Release of Information form required as part of your TVO ILC application. Instead, you can fill in the bottom row of the table just above the Signature and Date lines with your name and the date, as shown in the example below.
